- G6PD results
- Serum uric acid level within 1 month for initiation of therapy, within 48 hours for ongoing treatments
- Documentation of initiation of Methotrexate and folic acid
- Demographics
- Labs & Tests Supporting Diagnoses & Office/Progress Notes
